Health Care Directives
What is it? A written legal document in which a competent adult can declare health care instruction in the event of future inability to make decisions. The St. Cloud hospital offers volunteers, available free of charge, at the St. Cloud Whitney Center on the first and third Tuesday of each month from 11:00-12:00 to answer questions and help assist you in completing the forms.
